>>William D. Chey: We have some — several special programs at the
university that I participate in. One is called the Michigan Bower Control Program
and this is a program that I’m a codirector of with Dee Fenner and it’s a program
that I’m particularly proud of. It’s a multidisciplinary program that manages
patients with very severe chronic constipation or fecal incontinence and these are disorders
which have profound affect patient’s quality of life, ability to function and costs the
healthcare system tremendous amounts of money. And unfortunately, in the community,
oftentimes more severe cases of constipation and fecal incontinence, people have a great
deal of difficulty managing these patients because their management requires
a multidisciplinary approach. Probably only a few places like the University
of Michigan can really offer the breadth of comprehensive services that are necessary to adequately manage this
difficult group of patients. So, for example, in the Michigan
Bowel Control Program, we not only have specially trained expert
gastroenterologists but also urogynecologists, colorectal surgeons, physical therapists
and nutritionists that work specifically with our program to provide really
state of the art evidence based care for this very difficult to manage group. Many patients travel from long distances to
see us in these very specialized programs and I think that some of the things that
we have in place to help in the care of these patients are specific nurse concierges
that help to hold a patient’s hand and walk them through the patient care experience
at the University of Michigan. In addition, we’ve identified
ancillary providers. For example, nutritionists, physical
therapists in various sites throughout the State of Michigan and indeed, throughout the Midwest
that we can work with with a referring physician and also with those local ancillary
providers to be able to continue care even when patients can’t practically come back
over and over to the University of Michigan.
